AND CONDITIONS - Valid only with purchase
of
Circa Telecom digital solid-state modules. Each offer
good for ONE rebate per invoice. Rebate submission
must be postmarked within 30 days of the invoice. Rebate
value will not exceed purchase price.
Circa Telecom reserves the right to refuse rebate on any
damaged or non-functioning module submitted for rebate.
All modules and materials submitted become |
the property of Circa
Telecom, USA. Not responsible for submissions lost, misdirected,
or damaged by postal or delivery service. Void where taxed,
prohibited or restricted by law. Fraudulent submissions
may result in federal prosecution for mail fraud (Title
18 of the USC, sections 1341 and 1342).
Requests with invalid or undeliverable mailing addresses
will be denied. All rebates will be satisfied via company check. |
If rebate is not received
within 5 weeks, e-mail
rebate@circatelecom.com or call 800-783-6556. Claims
of non-payment will require photocopy substantiation
of all rebate documents including shipping or tracking
information. Offer valid in the U.S. and Canada.
